Join the Team - How it works

Step 1: Sign Up & Upload Credentials

Create an account with HMR Central by providing your pharmacist accreditation and contact details. This grants you access to the referral service that connects you with local GPs and clinics in need of home medication reviews for their patients.

Step 2: Receive Referral Alerts via SMS or Email

When a GP initiates a HMR request, our referral service instantly matches the request with an accredited HMR pharmacist in the area. You’ll receive an alert via SMS or email when a referral is available.

Step 3: Accept Referral via SMS or Email

Pharmacists can easily reply via SMS or email if they are able to accept the referral.

Step 4: Conduct HMR & Upload Review

The pharmacist conducts the home medication review and emails the report to HMR Central team where the GP can access.

Step 5: Submit Claim via the PPA Portal

Once the review has been uploaded, the pharmacist can lodge a claim for the HMR through the PPA Portal. A small fee is applied to each request accepted.
