How to refer

Step 1: Identify Eligible Patients for a HMR

Identify patients that have "a chronic medical condition and not having their therapeutic goals met" are eligible

Step 2: Send Your Referral Online

Submit your referral using your preferred method—email or fax

Step 3: Instant Pharmacist Match

Our platform immediately matches your referral with a qualified HMR pharmacist, who will arrange a comprehensive home medication review with your patient.

Step 4: Pharmacist Conducts Review & Sends Report to GP

The accredited pharmacist completes the review and sends their detailed report directly to you. You can then evaluate their recommendations and adjust the treatment plan as needed.

Step 5: GP Discusses Review with Patient & Bills MBS Item 900

The GP recalls the patient to discuss the review findings and updates the treatment plan. The consultation is billed under Medicare Benefits Schedule (MBS) Item 900.
