What is a home medicines review?
A home medication review is a personalized service that allows patients to have their medications reviewed by a specialist pharmacist, all from the comfort of their own home.
When referred by your general practitioner (GP), our expert team will carefully review your current medications—prescriptions, over-the-counter drugs, and supplements—to ensure they are safe, effective, and aligned with your health needs.
During the review, we check for any potential interactions, side effects, or duplications in your regimen, and provide you with clear guidance on how to optimise your treatment plan.
The goal is to improve your medication management, enhance your health outcomes, and give you confidence in your treatment—all in a convenient and accessible way.
What is the process for a HMR?
Step 1
GP identifies patients who may be suitable for a Home Medicines Review (HMR). This may include patients who are older, taking multiple medications, recently discharged from hospital, or not achieving the expected results from their current treatment plan.
Step 2
GP submits a HMR referral to HMR Central with the consent of the patient.
Step 3
An accredited pharmacist arranges a confidential consultation with the patient. During this appointment, they review all medications being used, discuss interactions, optimising medicines, storage and administration practices, and provide practical guidance where needed.
Step 4
GP receives the pharmacist’s comprehensive report and discusses the recommendations with the patient. Together, they agree on any necessary adjustments to the medication management plan. Ongoing communication and follow-up with the pharmacist can be coordinated.
Step 5
GP submits a claim under the Medicare Benefits Schedule Item 900 for the completed service
